Program Volunteer Sign-up Form

Note: The information on this form will be received by the Program Coordinator. The purpose of this form is to collect information on availability and preferences to match you with your LGBTQ seniors. Information submitted on this form will not be shared or distributed. We are excited about your interest in the Friendly Caller Program.

As a volunteer caller you will be matched with one or more LGBTQ seniors to make weekly calls. By signing up, you are making a commitment to dedicate at least 30 minutes per week to calls. After submitting this form, you will receive a call from the Program Coordinator to discuss training and next steps. Thank you for your support.
