We’re so pleased that Empath Health’s AIDS Service Association of Pinellas (ASAP) and BayCare Health System’s Pinellas Care Clinic (PCC) have partnered to create HOME 3050 – the Tampa Bay area’s first-of-its-kind medical home serving those with HIV.

 

Expertise & Enhanced Service

 

ASAP and PCC are two longtime, highly-respected nonprofit HIV service and care providers dedicated to serving our community and improving their quality of life. Currently PCC provides HIV care integrated with primary care to approximately 1,200 HIV-positive individuals. ASAP provides medical case management and other social services to nearly 1,000 individuals and eligibility determination for 1,800 HIV-positive individuals.

 

HOME 3050 is a tremendous opportunity for us to combine our expertise, expand and best serve our patients and clients, and eventually the community at large. Our mission is to deliver extraordinary full-service health care and social services to participants all on one convenient campus, located at 3050 1st Ave. S. in St. Petersburg. Renovations are moving along and we expect to open this fall.

 

HOME 3050 Services

 

HOME 3050 will offer:

 

  • Primary and HIV specialty care
  • Laboratory, pharmacy and benefits services
  • Counseling
  • Medical case management
  • Food and personal needs pantry
  • HIV and STD testing
  • Therapeutic and support groups
  • ‘Ryan White’ program eligibility determination
  • Education that focuses on prevention and wellness

 

The pharmacy services, provided by the Empath Health Pharmacy, will benefit HOME 3050 participants and beyond. All of the pharmacy’s proceeds will be reinvested into our community HIV services.

 

Care with Compassion

 

At HOME 3050, our personalized care teams will provide the highest level of patient-centered care and services with compassion, dignity and respect. Participants will be greeted by our care coordinator who will begin their intake and assist in identifying their needs. Teams may include a physician, nurse, case manager, counselor, pharmacist or other members committed to supporting participants’ needs and achieving their HIV treatment success. Our goal is to provide seamless care experiences that help everyone we serve live even healthier lives.
